Our Team
Sharon Armstrong
Sharon is our office Nurse Practitioner Complex Care Manager (NP-CCM). She is in the office 1-2 days per week, and specializes in woman‘s health. She is a fantastic resource for patients. Although she mainly practices in our office as an NP, she is a great provider for any patient who has concern that they are not being heard by their current provider or who wants a provider that focuses on counseling them regularly on their individual health goals. Sharon helps cross gaps to get patient’s access to the resources they need on a first hand basis as a Care Manager and Nurse Practitioner. Betty Falecki
Betty is a Certified Care Manager and is able to coordinate your care, help you set goals and manage chronic diseases such as hypertension, diabetes, COPD and depression with an individualized patient centered care plan. Autumn Whitten
Autumn is a Certified Physician Assistant and Care Manager who is available four days per week for acute and chronic condition management. Andria Janski
Graduated with a Masters in Physician Assistant studies from Massachusetts College of Pharmacy and Health Science in Manchester, New Hampshire.
Graduated with a Bachelor of Science in Exercise science with an emphasis on Preventative Cardiology from Central Michigan University.
Graduated high school from a small rural town on the East side of Michigan called Algonac High school which was just a ferry ride away from my childhood home on Harsens Island MI.
My work experience is diverse in both the roles I played and the location of the practices. My broad clinical experience gives me a unique way of approaching Medicine and providing care.
Paula Carnaghi
Paula is a Care Manager Pharmacist that is available in our office if you are struggling to pay for your medicines, have any questions about insurance coverage, signing up for medicare Part D, or anything else medication related. She can meet with a patient over the phone or face to face and is available in the office on Monday from 8am-noon. Care Management is here to provide non-judgmental help for all patients at our practice. If you have any questions please call her at 810-987-6200 ext. 7075. Practice Responsibilities
Listen to patients’ feelings and questions and help them make decisions about their care.
Keep treatments, discussions, and records private.
Provide 24-hour access to medical care and same-day appointments whenever possible.
Provide instructions on how to meet patients’ healthcare needs when the office is not open.
Care for patients with the best of the provider’s abilities based on the understanding of current evidence-based practice.
Coordinate plan of care with a Care Manager so patients know what to do when they leave the office.
Give patients clear directions about medicines and other treatments.
Send patients to trusted experts if needed.
Patient Responsibilities
Ask questions, share feelings, and be part of the care.
Be honest about history, symptoms, and other important information about personal health.
Tell provider about any changes in personal health and well-being.
Take all medicine and follow provider’s orders.
Make healthy decisions about personal daily habits and lifestyle.
Prepare for and keep scheduled doctor visits. Reschedule visits in advance whenever possible.
Call the office first with all problems, unless it is an emergency.
End every office visit with a clear understanding concerning shared expectations, treatment goals, and future plans.
Bring all medication and herbal supplement bottles to every appointment.
